Code Compliance Documents

Reservation/Lock Documents
The Lender will print the OHFA Loan Confirmation and the OHFA Lender's Review Certification upon reservation/lock.

Certification & Approval Documents
In the order listed below in one PDF attachment, the Lender uploads the following documents to the loan via the OHFA lender portal for certification and emails OHFA a notification of the upload at [email protected]:

• OHFA Loan Confirmation
• Executed OHFA Lender's Review Certification
• Loan Transmittal/1008
• URLA (Uniform Residential Loan Application)
• OHFA Residence History Form (three years reporting required for Non-Targeted residence/mortgagors)
• Loan Estimate
• OHFA Partial Exemption Disclosure - Second Lien - (Application time)
• Purchase Contract

If there are conditions on a loan, they will be emailed to you from OHFA.  You will meet all the conditions at one time, and in one pdf attachment, upload those conditions to the loan via the OHFA lender portal for certification and email OHFA a notification of the upload at [email protected].

OHFA provides these documents to Lender upon Loan Approval:

• OHFA Approval/Award Letter
• OHFA Second Lien DPA Letter
• HSP Enhanced DPA Letter (on HSP loans only)
• OHFA MRB Disclosure
• OHFA Mortgagor Closing Certification

Closing Documents
The lender completes the following documents prior to Closing for mortgagor(s) to sign:

• Mortgagor Affidavit and Certification Part 5
• Tax Exempt Financing Rider
• Notice Potential Recapture Tax - Part II
• OHFA Partial Exemption Disclosure - Second Lien - (Closing time)
• OHFA Subordinate Mortgage
• OHFA Promissory Note
• OHFA Future Contact Form
• OHFA Mortgagor Closing Certification (sent to the lender at approval)
• Deed Restriction for Enhanced DPA (HSP ONLY)

The Lender will upload the following documents, in the order listed below and in one PDF attachment, to the loan within 5 business days after loan closing via the OHFA lender portal and emails OHFA a notification of the upload at [email protected]:

• Copy of executed First Mortgage Closing Disclosure
• Copy of executed OHFA Second Lien DPA Letter
• Copy of executed OHFA Mortgagor Closing Certification
• Copy of executed OHFA Partial Exemption Disclosure
• Copy of executed OHFA Subordinate Mortgage (Second Lien)
• Copy of executed OHFA Promissory Note
• Copy of executed Deed Restriction for Enhanced DPA (HSP Program)
